A chapter of The Phylaxis Society may be constituted when seven or more members of the Society, all in good standing with their Masonic lodge, request an authorization to be constituted. To form a chapter of the Society, the package containing the following should be sent to the Executive Director for Chapters:

Chapters of The Phylaxis Society mirror the goals and purposes of The Phylaxis Society insofar as the resources of the chapter allow and insofar as such goals and purposes may be appropriate to a local chapter. It will be the primary goal of chapters to encourage research into the history of Prince Hall Masonry. A secondary but essential goal of chapters will be to promote goodwill toward the Society and to encourage eligible Masons to join our ranks.

Chapters will elect a chapter president, first vice president, second vice president, treasurer, and secretary. Chapters are encouraged to appoint three chairmen with the following responsibilities:
